A 45-year-old lady presented with pain in the abdomen. USG and CT scan showed a large Tumor in the neck of the pancreas which was almost invading the portal vein. The patient was advised of surgery which was very high risk due to the involvement of vessels. There was also a dilemma as to whether this would be possible by a Whipples procedure or a distal pancreatectomy. The patient was then referred to us given portal vein resection (HPB and Liver Transplant surgeon). It was a difficult surgery. However, we were able to separate portal vessels from the Tumor. We performed a subtotal pancreatectomy with splenectomy. The patient’s Postop course was smooth without any major complications. The patient was very satisfied with the treatment.
