Patient Profile:

Presenting Complaint:
- Pain in the right upper abdomen
Diagnostic Imaging:
- CT and MRI showed an 18 cm cystic lesion in the right lobe of the liver, reaching the left lateral segment and splaying the left hepatic veins. Right and left hepatic veins were not visualized.
36 year old gentleman presented with pain in RHC. CT MRI showed an 18 cystic lesion in the Right lobe of the liver reaching the left lateral segment splaying the left hepatic veins. Right and left hepatic veins were not visualized. the patient was referred thus given a possible need for liver resection. The patient was investigated with a PET scan which showed no evidence of disease anywhere else in the body. Possibly cystadenoma/carcinoma. AFP CEA CA 19.9 normal. We went ahead with almost an extended right hepatectomy preserving the branches of the left hepatic vein. Postop HPR turned out to be a grade 2 Neuroendocrine Tumor. Margins free. A DOTA scan was done later which showed No evidence of disease anywhere else in the body.
This case demonstrates the surgical management of a cystic lesion in the liver, which turned out to be a grade 2 Neuroendocrine Tumor. The successful surgery, coupled with postoperative imaging showing no evidence of disease elsewhere, highlights the importance of thorough investigations and multidisciplinary management in treating complex liver lesions. Regular follow-up and monitoring are essential for long-term outcomes.
